Safety experts warn not to put your dog’s name on their ID tag. Knowing the your dog’s name may help a thief pass on your dog to the unsuspecting new owners because it appears the thief knows your dog (because the dog responds to their name). Instead, my mommy had the following printed on my ID tag and recommends the following:

My mommy attached my rubberized id tag with her contact info (phone number) hidden from strangers yet easily accessible if I were lost.
